Medical Information Form (2 COPIES FOR ALL PARTICIPANTS INCLUDING ADULTS)

The following forms are due within TWO WEEKS of your work camp, unless otherwise stated:
Leaders' Covenant (Print, sign, and return ONE MONTH before camp - ALL Adult Leaders)
Adult Leader & Camper Background Check Verification (to be filled out by JUNE 1 by a church admin or lead pastor)
JP Participant Roster (to be filled out by the youth leader for EACH participant including adults)
Parent-Provided Information for Student Dietary/Medical/Special Needs (Send this form to parents. We only need it back if there is a need!)
Adult Leader Skill Level & Information Form
Transportation Information Request Form
