Upon registering a trip with JP, churches must complete the following online forms within 14 days of registering: 

1. Cancellation & Participation Policy

2. Child Protection Policy

3. Youth Group Covenant Form

4. Youth Group Photo Release Form

Two weeks before your trip, the following online forms must be completed by the trip leader: 

1. Participant Roster (to be filled out by trip coordinator for ALL participants)

2. Transportation Information

3. Adult Leader Background Check (to be filled out by a church admin)

4. Adult Skill Level & Information Form (send to adult leaders to have them fill out)

5. Dietary/Medical Needs Form (to be filled out by a parent ONLY if there is a specific need)

Below are the forms that each participant attending a Multi-Church week will need to fill out in advance and give to the trip leader:  

1. 3 Copies of Medical Information Form (must be completed by all Student & Adult participants)

Below are the forms that each participant attending Family Camp will need to submit upon arrival. 

1. Medical Information Form - We need 3 printed copies of this per participant!
