Forms to Bring to Camp

Youth Group Covenant & Photo Release Forms should be signed by a Youth Leader and Senior Pastor on behalf of all students

Medical Permission Form should be completed by all Student & Adult participants.  

Please bring two additional copies, for each participant, of these medical forms to camp


Due Date: All forms should be turned in to your JP Camp Director at the time of your arrival to camp
